Use Clubspot’s new custom form tool to create any number of questionnaires under Website > Forms. First, you’ll assign a name and optional description to the form. Then you’ll begin to add custom fields to build out your form. Use the circle + icon to the right of the document’s name to add a new custom field. You’ll then be able to use the dropdown menu to configure the type of response you’d like to collect.
If you’d like the form NOT to be anonymous, turn on the ‘data collection’ toggle to collect the user’s name and contact information.
The Responses tab will allow you to view and export all form responses, with filtering options for specific custom fields.
<aside> 💡 Custom forms can be used to collect data for a number of use-cases. Some examples include work orders, member surveys, volunteer signups, board elections, crew bank submissions, concierge requests, storage haul/launch times, etc.

Each form has a ‘settings’ tab where you’ll find a URL that can be shared directly, put in an email, added to an app, linked on a website, etc.
Alternatively, if you’d prefer to embed the form directly onto a webpage, you can do so in the web builder. Simply “+ new page element” and choose Form. Then you’ll have the option to choose which from you’d like to display on the body of your webpage. No need to embed a Google form ever again!
Under the ‘settings’ tab on each form you’ll be able to set up an email notification. Any number of email addresses can be configured to receive a notification upon form submission.
